Treatment Notes

We document relevant health information provided to us during physiotherapy treatment session.

Class registration/health questionnaire - To determine suitability for classes and exercise specific health related personal information will be obtained and documented.

External Medical Records

With your consent, we occasional obtain relevant medical records for other health professionals to assist with you treatment.

Financial Information

We record details regarding private medical insurance (if applicable) for invoicing.

All transactions made with card payments are recorded

We collect and record sensitive data regarding you health to assist with assessments and suitability for treatment interventions.

Personal Data is collected from yourself. The data is collected during bookings, pre-class screenings and during consultations. We obtain this information in person, over the telephone, via email, private messaging on social media or through our website.

When visitors leave comments on the site we collect the data shown in the comments form, and also the visitor’s IP address and browser user agent string to help spam detection.

An anonymised string created from your email address (also called a hash) may be provided to the Gravatar service to see if you are using it. The Gravatar service privacy policy is available here: https://automattic.com/privacy/. After approval of your comment, your profile picture is visible to the public in the context of your comment.
